What does outpatient hospice care look like?
Each out-patient receives dedicated attention from a team of professionals, including nurses, social workers, trained volunteers, and spiritual companions. We recognize the critical needs of our patients and their families and ensure a comprehensive support system.
In certain situations, our in-home patients may find respite in our facility for family relief or stabilization. Following this period, they typically return home to continue receiving the personalized care and comfort within the familiar surroundings of their residence. Our goal is to maintain a continuum of compassionate care, tailoring our services to the unique needs of each individual and family.
Who qualifies for outpatient hospice care?
Hospice care is tailored for individuals with a life expectancy of six months or less, providing specialized support to those with terminal illnesses. This includes people receiving care at home when their condition surpasses the family's capabilities and individuals recently discharged from the hospital due to disease progression. Hospice professionals focus on managing symptoms, enhancing quality of life, and offering emotional support during the final stages of life, aiming to provide comfort and dignity for both patients and their families.
